With reference to your request for proposal, we are pleased to submit the the following formal quotation for equipment located at your Pfizer Pearl River facility to be sent to Pfizer, Puurs, Belgium, per South Eastern Millwright and Riggings (SEM) estimate SEM will remove equipment from the Pfizer facility in Pearl River, NY as directed by Larry Kadis from Federal Equipment and provide transportation to Pfizer, Puurs, Belgium. All work will be performed in a professional manner for a lump sum price of: $40,250 US Dollar. SEM to be responsible for the following: 1. Provide equipment and rigging materials as needed. 2. Remove Pfizer equipment and load onto SEM air ride vans. 3. Trucking offload in SEM's Dover Facility. 4. Export Crate Method 2 (vacuum seal), and shock mount equipment as necessary for shipment. 5. Equipment will be loaded, blocked and braced, and drayage to NY Port. (Please note we have estimated (2) 40ft. sea containers based on our facility walk through.) Notes; SEM will provide all paperwork, Commercial Invoice, bills of lading, and packing list. Equipment transported Ocean Freight to Antwerp, Belgium, delivery in Puurs, Belgium. Our lump sum price (does not) include any NY and NJ permits, sales tax and/or use tax on labor and materials. This proposal is based on a standard work week for our scope of work at the date of this proposal. No premium time has been considered in our lump sum price. Any additional work above contract will be performed on a Time and Material basis.
